Overview
Supermove’s new Lead Forms feature allows companies to create multiple, customizable forms to capture lead details that generate projects directly in Supermove.
This is the easiest way to add a branded, custom lead capture form to your moving company’s website.
Locating Settings
A new Lead Forms section is now available under Settings → Sales
By default, every account includes the Move Request Form, which matches the standard Supermove lead form

From this portal, companies can:
Manage all lead forms
Track the number of leads captured by each form
Create a Lead Form
To create a custom lead form:
Click Create Lead Form in the top-right corner

Enter the form details, including:
Name
Description
Branch – which branch should own the new project
Project Type
Job Type
Project Tags (optional)
Job Tags (optional)
Click Save to create the form and open the Form Builder
Form Builder
The Form Builder works similarly to the Document Builder:
Left side: Add and configure form items
Right side: Live preview of your form
You can:
Add items using Add Form Item
Configure settings for each item type
Save a draft or Publish updates to the live form link
Add Form Items
Form items are grouped into three categories:
1. Form Elements
Basic inputs like Text, Image, and Space
Useful for formatting and adding display-only content
2. Form Inputs
Primary tools for capturing customer information
Variable Input: updates billing variables on the project (e.g., for auto-quoting features)
Other inputs: capture customer details, stored in Office Notes on the project
3. System Inputs
Special inputs that can only be added once per form

Used to update structured project and client details:
Customer Information – name, phone, email
Move Information – preferred date, time, move size
Locations – origin, destination, stops
Referral Information – source and details
Additional Notes – free text details
Next Step: Once your Lead Form is created, the final step is to share it with your customers. You can do this by linking directly to the form or embedding it on your website.

⚠️ Note About Custom Form Inputs
Custom Form Items (like text fields, dropdowns, or multi-selects) only send the answers into the project — their labels/questions do not appear anywhere in Supermove today.
If you need both the question and the answer to show in the project, use System Inputs or Variable Inputs, as these map cleanly into structured fields.
Our team is actively working on improvements to support labeled custom fields.
